Article 8 – Professional Responsibility provides a problem solving process for nurses to address

concerns relative to patient care.  This form is intended to appropriately identify employee

concerns relative to their workload issues in the context of their professional responsibility.

These issues include but are not limited to: gaps in continuity of care, balance of staff mix, access

to contingency staff and appropriate number of nursing staff.   This report form provides a tool

for documentation to facilitate discussion and to promote a problem solving approach.

PROFESSIONAL RESPONSIBILITY FORMS (workload forms)

  • The workload report form in itself is not a grievance, but the start of a paper trail to

identify and demonstrate ongoing problems that will require a response by your employer.

  • It also acts as proof of your identification of patient safety issues to your manager, and provides

you with documentation, should you find yourself in the unfortunate position of responding to

the Complaints and Disciplines Committee at the College of Nurse's .

  • The Local collects the information, and copies are given to the Labour Relations Officer.

  • Summary reports are presented and discussed at the Corporate Joint Hospital Association Committee .

  • It does work!  ONA has been very successful at implementing changes at the hospital level...but it starts with YOU!  We need the documentation to initiate the change.
  • If there are no forms on your unit, please notify your unit rep. or a member of the executive

  • You may download a copy of the form (above) if no form is available

  • Please photocopy the completed downloaded form, and give one copy to your manager and the other to your unit rep. or a member of the executive

  • If you are using the duplicate forms found in your unit, please give your manager the top copy and give the remaining copies to your unit rep. or executive.
